Information we request

Nomination forms collect the nominee’s name, grade or educator role, school or homeschool status, relevant achievements and impact, plus the nominator’s name, email, and relationship. Nominators may optionally upload one photo they have permission to share. Uploading a photo is not consent to publish it. Contact and sponsorship inquiries collect the information needed to respond.

How information is used

HomeTown Legacy uses submissions to review eligibility, assess nominations, contact nominators, administer recognition, and seek publication permission. Nominator contact information is not intended for public profiles. Nomination data is not a marketing subscription.

Separate consent before publication

Submitting a nomination is not consent to publish. We will privately verify eligibility and seek the appropriate guardian or adult nominee permission before publishing a profile, photo, school details, or personal story. Consent documents and student verification materials should not be sent through the public nomination form.

Please minimize sensitive information

Do not submit Social Security numbers, banking or card information, passwords, medical records, detailed hardship records, student identification documents, or private school records. Describe achievements respectfully and omit information you are not authorized to share.

Access & service providers

Submissions are intended for authorized HomeTown Legacy administrators and reviewers. Website and form service providers process information to operate these services. A voting service’s privacy terms will be identified before voting opens. Data must not be shared with sponsors for marketing without separate permission.

Requests & corrections

To ask about a submission, correct information, request withdrawal before publication, or discuss a published profile, email hometownlegacy@yahoo.com. Identity or authority may need to be verified privately before acting on a request.
